Nasolabial Folds: Filler, Sculptra or Both? Glow Theory Aesthetic & Dermatology Studio, Carlsbad

If you only read one thing

Nasolabial folds deepen with cheek volume loss, collagen loss and bone changes. Hyaluronic acid filler softens them immediately and lasts about 6 to 18 months (Vollure XC lasted up to 18 months in its FDA study); Sculptra, FDA-approved for nasolabial fold wrinkles in 2009, builds collagen over 2 to 3 sessions and can last up to about 2 years. Restoring cheek support often softens the fold more naturally than filling the line itself, and many people combine both.

  • Hyaluronic acid filler works immediately and lasts about 6 to 18 months.
  • Sculptra builds collagen gradually and can last up to about 2 years.

  • Vollure XC, Ultra Plus XC, Refyne, Defyne, Restylane-L, Versa and Radiesse are used here.
  • The area has important blood vessels; choose an experienced injector.
  • Botox is rarely the right treatment for these folds.

Nasolabial folds are the lines that run from the sides of the nose to the corners of the mouth. Everyone has them, and they deepen with age as the cheeks lose volume and skin loses collagen. The two main treatments are hyaluronic acid filler, which softens the fold straight away and lasts about 6 to 18 months, and Sculptra, which builds your own collagen gradually over a few sessions and can last up to two years. Many people get the best result from both, often starting with support in the cheeks rather than filling the fold itself.

This guide explains what causes nasolabial folds, how filler and Sculptra compare, why the cheeks matter, the fillers approved for this area, and what to expect.

What causes deep nasolabial folds?

  • Cheek volume loss: fat pads in the mid-face shrink and slide down, adding weight above the fold.
  • Collagen and elastin loss: thinner, less springy skin folds more deeply.
  • Bone changes: the bones around the nose and upper jaw recede with age, reducing support.
  • Sun damage, smoking, weight changes and genetics.

Smiling does not cause the fold, and Botox is rarely the right tool here; see Botox or filler for smile lines?

Filler vs Sculptra for nasolabial folds

Hyaluronic acid filler

Immediate softening

  • How it worksA gel adds volume under the fold or in the cheek
  • ResultsVisible right away; settles over 2 weeks
  • LastsAbout 6 to 18 months, depending on the product
  • ReversibleYes, with hyaluronidase

Sculptra

Gradual collagen

  • How it worksPoly-L-lactic acid stimulates your own collagen
  • ResultsBuild over 2 to 3 months per session; usually 2 to 3 sessions
  • LastsUp to about 2 years
  • ReversibleNo; results fade gradually

Why treating the cheeks often works better

Filling a deep fold directly can look heavy or flatten your smile, especially if the real problem is lost cheek volume. Restoring support in the mid-cheek with a firmer filler such as Juvéderm Voluma or Restylane Lyft lifts the tissue above the fold, so a small amount of a softer filler, or none, is needed in the line itself. Sculptra works in a similar way by rebuilding overall volume. Read more in our guide to facial volume loss and what Sculptra is.

Juvéderm Ultra Plus XCHyaluronic acidThicker gel for deeper folds
Restylane Refyne and DefyneHyaluronic acidFlexible gels that move with expression; up to 12 months
Restylane-L, Revanesse VersaHyaluronic acidClassic fold fillers

Your injector chooses the product and placement for your anatomy; see Juvéderm vs Radiesse for how two of these compare.

What to expect

  • Treatment: usually under an hour; most fillers contain lidocaine for comfort.
  • After: mild swelling, tenderness and possible bruising for a few days.
  • Results: filler shows immediately and settles over two weeks; Sculptra builds over months.
  • Maintenance: touch-ups when results begin to fade, usually less filler than the first time.

Safety: the area beside the nose has important blood vessels, so injecting it needs an experienced injector who knows the anatomy. Seek help straight away if you notice severe pain, skin turning white, blue or mottled, or any change in vision after filler. Our guide to vascular occlusion after filler explains the warning signs.

Frequently asked questions

What is the best filler for nasolabial folds?

It depends on your anatomy. Flexible hyaluronic acid fillers such as Vollure XC, Refyne or Defyne are common, often combined with cheek filler for support.

Is Sculptra better than filler for nasolabial folds?

Sculptra gives gradual, longer-lasting collagen-based results; filler works immediately and is reversible. Many people use both.

How long does filler last in nasolabial folds?

About 6 to 18 months, depending on the product and your metabolism.

Can Botox fix nasolabial folds?

Rarely. The folds come from volume loss, and relaxing nearby muscles would affect your smile.

Why does filling my folds look heavy?

The cause may be cheek volume loss. Lifting the cheeks first often gives a more natural result.

Is nasolabial filler safe?

Yes in experienced hands, but the area has important blood vessels, so know the warning signs of vascular occlusion.
