Botox for Smile Line

In a hurry? Skim mode fades everything except the highlighted takeaways.

If you only read one thing

For the smile lines that run from the nose to the mouth (nasolabial folds), filler usually works better than Botox. The folds come mainly from volume loss and weaker skin support, not from one overactive muscle, so relaxing muscles would flatten your smile. Botox does help the smile lines around the eyes (crow's feet), lip lines and a gummy smile.

  • Smile lines from nose to mouth are nasolabial folds, caused mainly by volume loss and weaker skin.
  • Botox rarely helps these folds; relaxing the muscles enough would flatten your smile.
  • Botox does help crow's feet, lip lines, a gummy smile and downturned mouth corners.
  • Hyaluronic acid filler in the fold or cheeks is FDA-approved for nasolabial folds and lasts about 4 to 12 months.
  • Sculptra, Radiesse and skin tightening are longer-term or complementary options.
  • Smile line filler at Glow Theory is $790 – $990.

“Smile lines” usually means the folds that run from the sides of your nose to the corners of your mouth, known medically as nasolabial folds. Many people ask for Botox to soften them. In most cases, Botox is not the right tool: these folds are mainly caused by changes in volume and skin support, not by a muscle that can be relaxed. Filler, collagen-stimulating treatments and skin tightening usually help much more.

This guide explains what causes smile lines, where Botox does and does not help, how filler works, other options, and what each costs at our Carlsbad studio.

What causes smile lines?

Everyone has some fold between the cheek and the upper lip; it is part of a normal face and becomes more visible when you smile. Over time it tends to deepen because of:

  • Volume loss in the cheeks, so the cheek tissue sits lower and heavier over the fold
  • Less collagen and elastin, so the skin is thinner and less springy
  • Sun damage and smoking, which speed up skin aging
  • Weight changes and genetics, which affect how full the face is

Smiling itself does not “cause” the fold, and you should not stop smiling to prevent it. The fold is mostly a matter of structure, which is why it responds to treatments that restore support.

Does Botox work for smile lines?

Botox relaxes muscles, so it works best on wrinkles caused by repeated muscle movement, such as frown lines, forehead lines and crow’s feet. The nasolabial folds are different: they are a fold of skin and fat, not a line drawn by a single overactive muscle. Relaxing the muscles that lift the lip and cheek enough to flatten the fold would also flatten your smile, which is why Botox is rarely used here.

There are some smile-related areas where Botox does help:

  • Crow’s feetThe fan of lines at the outer eye when you smile, sometimes also called smile lines. Botox is FDA-approved for these.
  • Gummy smileA tiny dose in the lip-lifting muscle can lower the upper lip and soften the very top of the fold in some people; see Botox for a gummy smile.
  • Lip linesFine vertical lines on the upper lip can soften with a few units; see smoker’s lines.
  • Downturned mouth cornersBotox in the muscle that pulls the corners down; see DAO Botox.

How filler treats smile lines

Hyaluronic acid fillers are FDA-approved for moderate to severe facial wrinkles and folds, including nasolabial folds. There are two main ways to use them, and the best results often combine both:

  • Directly in the fold: a soft, flexible filler such as Restylane Refyne, or Defyne for deeper folds, placed under the line to lift and smooth it while letting you smile naturally.
  • In the cheeks: restoring cheek volume lifts the tissue that sits over the fold, which often softens it more naturally than filling the fold alone. Our guide to Juvéderm Voluma covers cheek filler.

Treatment takes about 30 minutes. Expect some swelling, tenderness and possible bruising for a few days; the American Academy of Dermatology notes these usually settle within 7 to 14 days. Hyaluronic acid filler can be dissolved if needed.

Filler safety

Filler should always be injected by an experienced medical provider. The rare but serious risk is filler entering a blood vessel, which can damage skin or, very rarely, affect vision. Seek help straight away for severe pain, skin that turns white, gray or blue, or any change in vision after filler. Never buy filler online or let an untrained person inject it.

Other treatments for smile lines

  • SculptraStimulates your own collagen gradually over a few sessions; results can last up to two years. Good for overall volume loss.
  • RadiesseA firmer filler that also stimulates collagen, useful for deeper folds.
  • Skin tighteningMorpheus8 or Sofwave improve skin firmness and texture around the mouth.
  • Daily skincareSunscreen and a retinoid slow collagen loss; they will not erase a deep fold but help prevent it deepening.

Frequently asked questions

Does Botox work on smile lines?

Not usually for the folds from nose to mouth. They come mainly from volume loss and skin changes, not one overactive muscle, and relaxing the lip and cheek muscles enough to flatten them would also flatten your smile. Botox does work well on crow's feet.

What is the best treatment for smile lines?

Usually hyaluronic acid filler, placed in the fold, the cheeks or both. Sculptra, Radiesse and skin tightening can add longer-term improvement.

How long does filler for smile lines last?

Typically about 4 to 12 months for hyaluronic acid filler, depending on the product and your metabolism.

What causes smile lines?

Volume loss in the cheeks, thinning skin with less collagen and elastin, sun damage, smoking, weight changes and genetics. Everyone has some fold there.

Can you get rid of smile lines without filler?

Sculptra, skin tightening with Morpheus8 or Sofwave, and daily sunscreen and retinoids can soften them, but a deep fold usually needs volume to improve noticeably.

How much does filler for smile lines cost?

At Glow Theory smile line filler is $790 – $990, or $672 – $842 for VIP members.

Is smile line filler safe?

It is generally safe in experienced hands. Swelling and bruising are common for a few days. The rare serious risk is filler entering a blood vessel, so seek help straight away for severe pain, skin color change or vision change.
