If you only read one thing

Fillers do not ruin your face when used in sensible amounts, in the right places, by an experienced injector. Problems such as an overfilled pillow face, migration or stretched lips usually come from too much filler, too often, or in the wrong layer. MRI studies show hyaluronic acid filler can remain for years, so topping up on a fixed schedule can build up volume. Conservative, spaced treatment with dissolvable filler keeps results natural.

  • Filler does not damage the face when used conservatively.
  • Overfilling causes the puffy pillow face look.
  • Hyaluronic acid filler can stay in tissue for years.
  • Top up by how you look, not by the calendar.
  • When filler wears off, you return to how you would have looked.
  • Very large volumes over years can stretch lip tissue.
  • Hyaluronic acid filler can be dissolved and rebuilt.

Fillers do not ruin your face when they are used in sensible amounts, in the right places, by an experienced injector. Most people who have filler regularly look like a refreshed version of themselves. The problems people worry about, a puffy, overfilled “pillow face”, filler drifting out of place, or lips that look stretched when filler wears off, usually come from too much filler, too often, or in the wrong layer. Research also shows hyaluronic acid filler can stay in the tissue much longer than the 6 to 18 months often quoted, so topping up before it has gone can build up volume over time. Choosing conservative treatment, spacing appointments and using dissolvable filler keeps the risks low.

This guide explains what fillers really do long term, the overfilled look and how to avoid it, what happens when you stop, and how problems are fixed.

Can fillers ruin your face?

Filler itself does not damage your face, and hyaluronic acid is a substance your skin already makes. What can make a face look worse is how filler is used. The common problems are:

  • OverfillingToo much volume in the cheeks, lips or mid-face can look heavy and puffy, a look often called “pillow face”, and can blur natural features.
  • Build-up over yearsIf filler is topped up on a fixed schedule while the old filler is still there, volume slowly accumulates.
  • MigrationFiller can drift from where it was placed, for example a ridge above the lip. See lip filler migration.
  • Wrong area or productFilling a jowl or a puffy under-eye can make it look worse; firm fillers in thin skin can show as lumps or a bluish tint.
  • Tissue stretchingVery large volumes over many years can stretch lip tissue, so lips may look looser once the filler goes.
  • Swelling and delayed reactionsSome people with a lot of filler notice puffiness, especially in the morning, or delayed swelling after an illness.

How long does filler really last?

Filler labels typically list how long the visible effect lasts, often 6 to 18 months depending on product and area. MRI studies tell a different story about what stays in the tissue. In a 2024 review of 33 patients, hyaluronic acid filler was still detectable at least two years after injection in every patient, and in some cases for many years. The visible lift fades sooner because the filler softens and spreads, but some product can remain.

The practical lesson is simple: top up based on how your face looks, not on the calendar. A good injector will tell you when you do not need more.

Signs you may have too much filler: cheeks that look wide or round from the front, a heavy mid-face when you smile, lips that stick out in profile or have lost their shape, puffiness that never quite goes away, or friends saying you look different rather than well-rested.

What happens when you stop getting filler?

Your face does not sag more because of filler. When hyaluronic acid filler breaks down, you return gradually to how you would have looked anyway, a little older because time has passed. Some fillers, such as Radiesse and Sculptra, also stimulate your own collagen, so part of the improvement can last after the product has gone. The one exception is very large volumes over many years, which can stretch tissue. See what happens when you stop lip filler.

How to keep filler natural for the long term

  • Start small and build gradually; you can always add, and it is easier than taking away.
  • Treat the cause: restoring support in the cheeks or temples often looks better than filling lines directly. See facial volume loss.
  • Space treatments and judge by photos, not a fixed schedule.
  • Mix treatments: Botox for movement, skin tightening for laxity and collagen builders for skin quality reduce how much filler you need.
  • Choose dissolvable hyaluronic acid filler in delicate areas such as lips and under-eyes.
  • Choose an injector who will say no when you do not need more.

Can overfilled faces be fixed?

Yes. Hyaluronic acid filler can be dissolved with hyaluronidase, which usually works within 24 to 48 hours, with the full effect in up to two weeks. Many people dissolve old or migrated filler, let the tissue settle, and then rebuild with a smaller, better-placed amount, or decide they do not need filler at all. See filler dissolver.
