What Happens When You Stop Getting Lip Filler?

If you stop getting lip filler, your lips gradually return to roughly how they looked before treatment. Hyaluronic acid filler is broken down naturally by the body, so there is no sudden change, no “crash” and, for most people, no lasting damage. The changes people worry about, such as stretched or saggy lips, are uncommon and mostly linked to very large amounts of filler over many years.

Here is what actually happens when lip filler wears off, how long it takes, why some filler can linger, and how to stop safely, including dissolving it if you want a faster reset.

What happens to your lips when filler wears off

• Volume fades slowly: your body breaks down hyaluronic acid little by little. Most people notice their lips looking softer from about 6 months, with visible filler mostly gone by 12 to 18 months, depending on the product, amount and your metabolism.
• Shape returns: definition along the lip border and a sharper Cupid’s bow fade with the volume.
• Hydration drops a little: hyaluronic acid holds water, so treated lips can look smoother and more plump. That sheen fades as the filler does.
• Your natural aging continues: if you had filler for many years, your lips may look a little thinner than you remember, simply because you are older, not because the filler made them worse.

Will my lips look worse than before?

Usually not. Most people’s lips return to their own natural size and shape. They can look smaller at first only because you are used to seeing them fuller. A few situations can make a difference:

• Very large volumes over many years can stretch the lip tissue, and some people notice looser or slightly creased lips once the filler goes. This is uncommon with moderate, gradual treatment.
• Filler that has migrated above the lip border (sometimes called a “filler mustache”) does not always fade evenly and may need dissolving.
• Lumps or uneven areas that formed during treatment may persist until they are dissolved.

Filler can last longer than you can see

Lip filler stops being visible after about a year, but some hyaluronic acid may stay in the tissue longer. In a 2024 review of MRI scans of 33 patients who had filler in the midface (not the lips), filler was still detectable in every patient two years later, and in some patients many years later. The authors suggested that refilling on a fixed schedule may build up more filler than people realize. This is one reason we prefer to top up lips only when they need it, not on a calendar.

Ways to stop lip filler

Dissolving works only on hyaluronic acid fillers such as Juvéderm and Restylane. Expect swelling for a day or two, and your lips may briefly look smaller or flatter than your natural lips while things settle. Caring for your lips after filler

• Protect them from the sun with a lip balm with SPF 30 or higher, since UV light breaks down collagen in the lips too.
• Keep them moisturized with a fragrance-free balm or petroleum jelly, especially in dry weather.
• Do not smoke, which thins the lips and deepens lines around the mouth.
• Consider gentler options: a Botox lip flip turns the upper lip out slightly for a subtle look without filler, and a small amount of soft filler can restore hydration without much volume.

A note on cold sores
