If you only read one thing
Lip filler migration is filler moving outside the lip, most often forming a puffy ridge above the upper lip border. It is usually caused by too much filler, high injection pressure, placement right at the border or frequent top-ups. Unlike swelling, it does not settle after two weeks. Migrated hyaluronic acid filler can be dissolved with hyaluronidase, which works within 24 to 48 hours, and the lips can then be rebuilt with a smaller, well-placed amount.
- Migration shows as a ridge above the lip line or a blurred border.
- Normal swelling settles within about two weeks; migration does not.
- Overfilling, pressure, placement and frequent top-ups cause it.
- Hyaluronidase dissolves migrated hyaluronic acid filler.
- Dissolver works in 24 to 48 hours; full effect up to two weeks.
- Rebuild with a smaller amount once settled.
- Gradual, low-volume treatment helps prevent it.

Lip filler migration is when filler moves outside the area where it was placed, most often above the border of the upper lip, creating a puffy ridge sometimes called a “filler mustache” or “duck lip” shelf. It is usually caused by too much filler, too much pressure or the wrong placement, and it can build up slowly over years of top-ups. Migrated hyaluronic acid filler can be dissolved with hyaluronidase, after which the lips can be rebuilt with a smaller, well-placed amount.
This guide explains what migration looks like, why it happens, how to tell it from normal swelling, and how it is fixed and prevented.
What lip filler migration looks like
- A puffy ridge or shelf above the upper lip, so the lip border looks blurred or “lost”.
- Fullness that extends beyond the lip line, especially when you smile.
- Lumps or firmness in the skin around the mouth, or inside the lip.
- Lips that look wider or more “duck-like” than intended, even after the filler should have worn off.
Migration or normal swelling?
Normal swelling
First 1 to 2 weeks
- TimingPeaks in the first 2 to 3 days
- LookOverall puffiness, sometimes uneven
- CourseSettles within about two weeks
Migration
Persists or builds
- TimingStill there after swelling settles, or develops over months
- LookA ridge above the lip line, blurred border
- CourseDoes not go away on its own quickly
See our guide to lip filler swelling stages for the normal day-by-day timeline.
Why lip filler migrates
- Too much fillerLarge volumes, or frequent top-ups before the last filler has worn off, stretch the lip and push gel outward.
- High injection pressureForcing filler in quickly can track it into the surrounding tissue.
- PlacementFiller placed too superficially or right at the border can spread above it.
- Muscle movementThe lip muscle moves constantly, which can shift filler over time.
- Product choiceSome gels are better suited to the lips than others.
- Filler lasting longer than expectedImaging studies suggest hyaluronic acid filler can persist for years, so repeated top-ups add up.
How migrated lip filler is fixed
- Assessment: your injector examines the lips and confirms the filler is hyaluronic acid; non-HA fillers cannot be dissolved.
- Dissolving: hyaluronidase is injected into the migrated filler. It begins working within 24 to 48 hours, with the full effect in up to two weeks. Some people need more than one session.
- Waiting: the lips are left to settle, usually for at least two weeks.
- Rebuilding (optional): a smaller amount of a suitable filler is placed carefully to restore shape and a crisp border.
Hyaluronidase can cause swelling and, rarely, an allergic reaction, so tell your injector about any allergies. See filler dissolver in Carlsbad.
How to prevent migration
- Choose an experienced injector who builds volume gradually, over more than one session if needed.
- Avoid frequent top-ups; wait until your lips need it.
- Avoid pressing, massaging or heavy pressure on the lips for the first day or two unless your injector advises it.
- Consider a lip flip if you want more show of the upper lip without extra volume.

Not migration: pain that gets worse, skin turning white, blue or mottled, or blisters after lip filler can mean a blocked blood vessel and need urgent care. See vascular occlusion after filler.

Frequently asked questions
What does lip filler migration look like?
A puffy ridge or shelf above the upper lip, a blurred lip border, or fullness beyond the lip line, especially when smiling.
What causes lip filler migration?
Usually too much filler, high injection pressure, placement right at the border, or frequent top-ups before old filler has worn off.
Can migrated lip filler go away on its own?
It may soften slowly as filler breaks down, but this can take a long time; dissolving is faster.
How is lip filler migration fixed?
With hyaluronidase to dissolve the filler, then, if wanted, a smaller amount of filler placed carefully after the lips settle.
How soon can I get filler after dissolving?
Usually after at least two weeks, once swelling has settled and the result of the dissolver is clear.
How do I prevent lip filler migration?
Choose an experienced injector, build volume gradually, and avoid frequent top-ups.
