Botox for Excessive Sweating: Underarms, Hands and Feet

If you sweat through shirts in an air-conditioned office, carry spare tops or avoid raising your arms, you are not alone. Excessive sweating, called hyperhidrosis, is a real medical condition, and it is not about being unfit or anxious. For underarm sweating, Botox is one of the most effective treatments available: in clinical trials, most people sweated at least half as much, and a single treatment typically lasted around six months.

What is hyperhidrosis?

Hyperhidrosis means sweating much more than your body needs to cool down. Primary hyperhidrosis usually starts in childhood or the teenage years, often runs in families and affects specific areas, most often the underarms, palms, soles of the feet or face. The sweat glands are normal; the nerves that switch them on are overactive.

Sweating that starts suddenly in adulthood, affects the whole body or happens at night can have another cause, such as a medicine, hormone changes or a medical condition. See your doctor if that sounds like you.

How does Botox stop sweating?

Sweat glands are switched on by a chemical messenger released from nerve endings. Botox blocks that messenger in the treated skin, so the glands there receive the “sweat” signal far less often. Botox reduces sweating rather than stopping it completely, and only in the area treated. The rest of your body still sweats normally to keep you cool.

For underarms, Botox is FDA-approved for adults with severe primary underarm sweating that is not controlled by topical treatments. In the clinical trials, around 80 to 90% of people treated had at least a 50% reduction in underarm sweat four weeks after treatment, and the median time before sweating returned was 201 days, or about six and a half months.

Botox for underarm sweating: what to expect

• BeforeShave the day before and skip deodorant on the day. We may do a starch-iodine test, which turns the sweating area dark purple, to map exactly where to inject.
• DuringMany tiny, shallow injections spread across each underarm, about a centimeter or two apart. It takes about 15 to 20 minutes; numbing cream or ice keeps it comfortable.
• UnitsThe FDA-approved dose is 50 units per underarm, 100 in total. Some people need more, up to about 200 units in total.
• AfterGo straight back to your day. Avoid deodorant, heavy sweating and hot baths for the rest of the day.
• ResultsSweating eases within 7 to 10 days, with the full effect at about two weeks.

Results vary from person to person. The American Academy of Dermatology reports underarm results lasting anywhere from 3 to 10 months. Most people book again when they notice sweating creeping back, often once or twice a year.

Side effects

Underarm Botox is a well-studied, low-risk treatment. The most common side effects are mild:

• Brief stinging and small bumps at the injection points
• Small bruises, which fade within a week
• Mild soreness, itching or tenderness in the underarm
• Occasionally, a little more sweating elsewhere on the body

Botox is not suitable during pregnancy or breastfeeding, or for people with certain neuromuscular conditions. Tell us about any medicines you take. Seek urgent care for trouble swallowing, speaking or breathing after any Botox treatment; this is very rare at these doses.

Botox for sweaty hands, feet and face

Botox can also reduce sweating on the palms, soles and face, but this is “off-label”, meaning it is not FDA-approved for those areas, even though dermatologists commonly use it. The American Academy of Dermatology reports results lasting 3 to 10 months for hands, 3 to 6 months for feet and about four and a half months for the face. Palm and sole injections are more uncomfortable, and in the hands there is a small risk of temporary weakness in the grip.

Does insurance cover Botox for sweating? Some plans do for severe underarm sweating, usually only after prescription antiperspirants have failed and with prior approval. Check with your insurer before your visit.
