If you only read one thing
Scalp psoriasis causes raised plaques with thick, dry, silvery scale on the scalp, often spreading past the hairline, and affects more than half of people with psoriasis. It differs from seborrheic dermatitis, which has thinner, greasy, yellowish scale. Salicylic acid and coal tar shampoos left on for 2 to 5 minutes help mild cases; prescription steroid or vitamin D solutions and foams, roflumilast foam, injections, light therapy and biologics treat more severe cases. Hair loss is usually temporary.

- Psoriasis scale is thicker and drier than seborrheic dermatitis.
- Leave salicylic acid or coal tar shampoo on for 2 to 5 minutes.
- Never scrape or pick off scale.

Scalp psoriasis causes raised, red or darker patches covered with thick, silvery-white or gray scale on the scalp, often spreading past the hairline onto the forehead, neck or behind the ears. It is common: more than half of people with psoriasis have it on the scalp. It is not contagious and is not caused by poor hygiene. Medicated shampoos help mild cases, and prescription scalp foams, solutions and newer treatments clear most moderate to severe scalp psoriasis.
This guide covers the symptoms, how to tell scalp psoriasis from dandruff and seborrheic dermatitis, what works at home, prescription treatment, and hair loss.
Symptoms of scalp psoriasis
- Raised, well-defined plaques with thick, dry, silvery or gray scale; on brown and Black skin, plaques may look purple or dark brown.
- Itching, burning or soreness, sometimes intense.
- Flakes that look like severe dandruff.
- Patches that extend past the hairline, onto the forehead, back of the neck and around the ears.
- Dryness, cracking and occasional bleeding from scratching.
- Temporary hair shedding in affected areas.
Many people also have psoriasis elsewhere, such as the elbows, knees, lower back or nails. Our guide to psoriasis in skin of color shows how it can look different on darker skin.
Scalp psoriasis vs dandruff vs seborrheic dermatitis
| Condition | Scale and look | Other clues |
|---|---|---|
| Scalp psoriasis | Thick, dry, silvery plaques with clear edges | Often past the hairline; psoriasis elsewhere or nail changes |
| Seborrheic dermatitis | Thinner, greasy, yellowish scale; pink skin | Also on eyebrows, sides of the nose and chest |
| Dandruff | Fine, loose white flakes, no thick plaques | Mild itch; clears with anti-dandruff shampoo |
| Scalp eczema or ringworm | Itchy patches; ringworm can cause broken hairs | Ringworm is contagious and needs antifungal pills |
The two can overlap (“sebopsoriasis”). Because treatments differ, a dermatologist can confirm which you have.
What helps at home
- Salicylic acid shampooSoftens and lifts scale so other treatments can reach the skin.
- Coal tar shampooSlows skin cell growth and eases itch and scale. It can stain light hair and has a strong smell.
- Leave it onLather onto the scalp, not just the hair, and leave for 2 to 5 minutes before rinsing.
- Alternate shampoosFor example salicylic acid one day and an anti-yeast shampoo the next, especially if seborrheic dermatitis overlaps.
- Soften scale gentlyAn oil or scale softener before washing, then a soft brush or comb. Never scrape or pick plaques off.
Avoid harsh hair dyes, tight hairstyles and very hot styling tools on inflamed scalp, and look for products with the National Psoriasis Foundation Seal of Recognition.
Prescription treatment
- Corticosteroid solutions, foams and shampoos: calm inflammation quickly; often used for short courses during flares.
- Vitamin D analogs such as calcipotriene, often combined with a steroid for long-term control.
- Roflumilast foam: a newer, once-daily, non-steroid foam approved for the scalp and body.
- Steroid injections into a few stubborn plaques; see intralesional injections.
- Light therapy with targeted devices, which reach the scalp through parted hair.
- Pills and biologics such as apremilast, deucravacitinib or IL-17 and IL-23 biologics for moderate to severe or widespread psoriasis, especially if it affects the joints.
Does scalp psoriasis cause hair loss?
It can cause temporary shedding, mostly from scratching, combing out scale and inflammation. The hair usually grows back once the psoriasis is controlled. Brush gently, avoid pulling off scale, and treat flares early. Scarring hair loss is rare, but see a dermatologist if patches of hair loss persist.
See a dermatologist if home treatment has not helped after a few weeks, if the scalp is very itchy, painful, cracked or bleeding, if you are losing hair, or if you have joint pain, stiffness or swelling, which can be a sign of psoriatic arthritis.
