If you only read one thing

Cystic acne is severe acne that forms deep, painful nodules and cysts under the skin and often scars. It rarely clears with over-the-counter products. Dermatologists treat it with an oral antibiotic plus a topical retinoid and benzoyl peroxide, hormonal therapy such as spironolactone or birth control for women, or isotretinoin, which clears acne permanently for about 85% of patients after one course. A cortisone injection can shrink a single painful cyst within days. Do not squeeze cysts.

  • Cystic acne forms deep nodules and cysts that often scar.
  • Hormones and genetics are the main drivers; it is not caused by poor hygiene.
  • Do not squeeze; ice, benzoyl peroxide and a warm compress once a head forms can help.
  • Antibiotics are paired with topicals and limited to about 3 to 4 months.
  • Spironolactone or birth control help hormonal cysts in women.
  • Isotretinoin clears acne permanently for about 85% after one course.
  • Cortisone injections shrink a single cyst within days.

Cystic acne is the most severe form of acne. Instead of surface pimples, it causes large, deep, painful lumps under the skin that can last for weeks and often leave scars. Because the inflammation sits so deep, over-the-counter washes and spot treatments rarely clear it. Cystic acne usually needs a dermatologist: prescription medicine, sometimes a cortisone injection for a single painful cyst, and a plan to prevent scarring.

This guide explains what cystic acne is, what causes it, what you can safely do at home for a painful cyst, and how dermatology treats it, from antibiotics and hormonal therapy to isotretinoin.

What is cystic acne?

Acne starts when a pore clogs with oil and dead skin and bacteria multiply inside it. In cystic acne, the clogged pore ruptures deep in the skin and the inflammation spreads, forming a large, tender lump. Dermatologists often call these nodules (firm, solid lumps) and cysts (softer, filled with pus); many people have both, which is why it is also called nodulocystic or nodular acne.

What causes cystic acne?

  • Hormones: androgens raise oil production. Cysts often flare in the teen years and, in adult women, around periods and along the jawline; see hormonal acne.
  • Genetics: severe acne tends to run in families.
  • Clogged pores and bacteria: the same process as other acne, but with a stronger inflammatory reaction.
  • Triggers that make it worse: picking and squeezing, heavy or pore-clogging products, friction from helmets or masks, some medicines, and stress. Our guide to why acne gets worse covers these.

Cystic acne is not caused by poor hygiene, and scrubbing harder makes it worse.

What to do about a painful cyst at home

  • Do not squeeze itCysts are too deep to pop. Squeezing pushes the inflammation deeper and raises the risk of infection, dark marks and scars.
  • Ice itHold ice wrapped in a paper towel on the spot for 5 to 10 minutes, a few times with breaks, to ease pain and swelling.
  • Use benzoyl peroxideA thin layer of 2% benzoyl peroxide helps kill acne bacteria. It can bleach fabrics.
  • Warm compress once a head formsA warm, not hot, washcloth for 10 to 15 minutes, three to four times a day, helps it drain on its own.
  • Skip the toothpasteAnd internet remedies. They irritate skin and can clog pores.

If a cyst is very painful or you have an event coming up, a dermatologist can inject it (see below). If you get cysts regularly, home care is not enough: treatment that prevents new cysts is what stops scarring.

How dermatologists treat cystic acne

Antibiotics plus a topical medicine

Usually the first step. An oral antibiotic such as doxycycline calms inflammation, while a topical retinoid and benzoyl peroxide unclog pores and fight bacteria. To limit antibiotic resistance, current guidelines keep oral antibiotics to the shortest time possible, ideally no more than 3 to 4 months, and continue the topical afterward.

Hormonal therapy

For women whose cysts flare with their cycle or sit along the jaw and chin, certain birth control pills or spironolactone, a medicine that blocks the effect of androgens on oil glands, can reduce breakouts. Results take a few months.

Isotretinoin

Isotretinoin (formerly sold as Accutane) is the most effective treatment for severe cystic acne and the only one that targets all four causes of acne. According to the American Academy of Dermatology, about 85% of patients see permanent clearing after one course, which usually lasts several months. Because it causes serious birth defects and needs monitoring, patients in the U.S. enroll in the iPLEDGE program and have regular check-ins.

Cortisone injections for a single cyst

A small dose of corticosteroid injected straight into a large cyst can shrink it and ease the pain within a few days, and sometimes within hours. It treats that one spot, not the acne itself, so it works best alongside a treatment plan. See cyst and keloid injections.

In-office treatments that support medicine

  • Forever Clear BBL: high-intensity blue light that targets acne bacteria and calms inflammation, with no downtime. It can help alongside prescriptions, but it does not replace them for severe cystic acne.
  • Chemical peels: help clear pores and fade dark marks between visits; see which peel to choose.
  • Acne facials with extractions are for blackheads and whiteheads. Cysts should never be extracted in a facial.

See a doctor promptly if a lump becomes hot, rapidly larger and very painful, or you have a fever, which can mean infection; if acne suddenly becomes severe with joint pain or feeling unwell; or if acne is affecting your mood or confidence. Help is available, and early treatment prevents scarring.

Preventing and treating scars

The best way to prevent acne scars is to treat cystic acne early and avoid picking. Once acne is under control, scars can be treated with subcision, RF microneedling, lasers and peels; see how to get rid of acne scars and laser treatment for acne scars. Brown and red marks fade with time and the right skincare routine.

Frequently asked questions

What is cystic acne?

The most severe form of acne, with deep, painful nodules and cysts under the skin that last weeks and often scar.

What causes cystic acne?

Mainly hormones and genetics, which drive oil production and a strong inflammatory reaction in clogged pores.

How do you get rid of a cystic pimple fast?

A dermatologist can inject it with a small dose of cortisone, which usually shrinks it within a few days. At home, ice it and do not squeeze it.

Should I pop a cyst?

No. Cysts are too deep to pop, and squeezing raises the risk of infection, dark marks and scars.

What is the best treatment for cystic acne?

It depends on the cause and severity: antibiotics with topicals, hormonal therapy for women, or isotretinoin for severe or scarring acne.

How long does cystic acne treatment take to work?

Most treatments take 2 to 3 months to show clear improvement; an isotretinoin course usually lasts several months.
