Botox for Jelly Roll

In a hurry? Skim mode fades everything except the highlighted takeaways.

If you only read one thing

Yes, in the right person. The jelly roll is a band of eyelid muscle that bunches up under the lower lashes when you smile. A tiny dose of Botox, often 1 to 2 units per side, relaxes it so the roll is softer. It suits people with firm lower lids and no under-eye bags; loose lids, bags or dry eyes make it risky. Results last about three to four months.

  • The jelly roll is muscle under the lower lashes that bunches when you smile, not fat or fluid.
  • A tiny dose of Botox, often 1 to 2 units per side, softens the roll.
  • Good candidates have firm lower eyelids, no under-eye bags at rest and healthy eyes.
  • Loose lower lids, under-eye bags, dry eyes or previous eyelid surgery make it risky.
  • Possible side effects include puffiness, dry or watery eyes and a droopy lower lid, all temporary.
  • Hollows and dark circles need filler or skin treatments, not Botox.

The “jelly roll” is the small roll of muscle that pops up just under your lower lashes when you smile or squint. Some people love it; in Korea it is prized as aegyo sal, a youthful, friendly look. Others feel it makes their eyes look smaller or puffy when they laugh. A tiny dose of Botox can soften it, but this is one of the most delicate places on the face to treat, and it is not right for everyone.

This guide explains what the jelly roll is, how Botox works on it, who is a good candidate and who is not, the risks, and what helps if your concern is actually bags, hollows or dark circles.

What is the jelly roll under the eye?

A ring-shaped muscle, the orbicularis oculi, surrounds each eye and closes it when you blink, squint or smile. The part closest to your lower lashes can bunch up when it contracts, forming a soft, horizontal roll just under the lash line. That is the jelly roll. It is a muscle, not fat or fluid, which is why it appears mainly when you smile and flattens when your face is relaxed.

It is easy to confuse with other under-eye concerns, and the difference matters, because each needs a different treatment:

ConcernWhat you seeWhat helps
Jelly rollA roll right under the lashes that appears when you smileA tiny dose of Botox, in the right candidate
Crinkly under-eye linesFine lines under the eye when you smileVery low-dose Botox, skin treatments such as MOXI
Under-eye bagsA puffy bulge lower down, present even at restNot Botox; this is fat and needs a surgical opinion
Hollows and dark circlesA shadowed groove between the eye and cheekUnder-eye filler such as Restylane Eyelight, or skin treatments

How Botox treats a jelly roll

Your injector places a very small amount of Botox into the thin muscle just under the lower lashes, usually one tiny injection on each side, a few millimeters below the lash line. This relaxes the band of muscle slightly, so it bunches up less when you smile. Small studies of low-dose Botox in the lower eyelid have shown softer under-eye lines and a slightly more open-looking eye.

Because the area is so delicate, the dose is tiny, often 1 to 2 units per side, and it is often combined with crow’s feet treatment for a smoother smile overall. We start low and check at two weeks; it is easy to add a little more and impossible to take it away.

Who is a good candidate, and who is not

  • Good candidateA visible roll when you smile, firm and springy lower eyelids, no under-eye bags at rest, and healthy, comfortable eyes.
  • Not a good candidateLoose or lax lower eyelids (we check with a gentle “snap” test), visible under-eye bags or puffiness at rest, dry eyes, previous lower eyelid surgery or laser resurfacing, or a lower lid that already turns outward.
  • Not suitable at allPregnancy, breastfeeding or certain neuromuscular conditions.

The lower eyelid muscle helps hold the lid against the eye and pump tears away. In people whose lower lids are already weak, relaxing it can make the lid sag, make under-eye bags look worse or cause watery or dry eyes. That is why the assessment matters more here than almost anywhere else on the face.

Results, timing and how long it lasts

Botox starts working in 3 to 7 days and reaches its full effect at about two weeks. Expect a softer, flatter roll when you smile, not a completely different eye shape. Because the doses are so small, the result usually lasts around three to four months, sometimes a little less than Botox elsewhere on the face.

Side effects and risks

  • Small bumps, redness or a bruise at the injection point; the under-eye skin is thin, so bruising is a little more likely
  • Puffiness or more noticeable under-eye bags, if the muscle relaxes too much
  • Dry, gritty or watery eyes, because the muscle helps spread and drain tears; a 2024 study linked Botox around the eyes with lower tear production
  • A slightly droopy or rounded lower lid, uncommon and temporary, but it lasts until the Botox wears off
  • An uneven look between sides, which can often be balanced at follow-up

Call us if your eye feels dry or irritated, waters constantly or does not close fully. Seek urgent care for any change in vision or trouble swallowing or breathing after Botox; these are very rare at cosmetic doses.

Other treatments for the under-eye area

  • Hollows and dark circlesUnder-eye filler can soften the groove; see our guide to dark circles under the eyes.
  • Crepey skin and fine linesSkin treatments such as MOXI or Morpheus8 improve texture.
  • Crow’s feetBotox at the outer eye, often treated together with the jelly roll.
  • Under-eye bagsA surgeon can assess whether lower eyelid surgery would help.

How much does jelly roll Botox cost?

Botox at Glow Theory is $14 per unit, or $11.90 for Glow Getter VIP members. Because the jelly roll needs only a few units, it is usually added to another treatment, such as crow’s feet, in the same visit. Your units and price are confirmed at your consultation. Our Botox units guide shows typical units for every area.

Frequently asked questions

Does Botox work for a jelly roll?

Yes, in the right candidate. A tiny dose relaxes the band of eyelid muscle just under the lashes so it bunches up less when you smile.

How many units of Botox for a jelly roll?

Usually a very small dose, often 1 to 2 units per side, frequently combined with crow's feet treatment.

Is under-eye Botox safe?

In carefully selected people, at low doses, it is generally safe. It is not suitable if your lower eyelids are loose, you have under-eye bags at rest, dry eyes or previous lower eyelid surgery.

Can Botox make under-eye bags worse?

Yes, it can. If the puffiness is fat rather than muscle, relaxing the muscle can let it bulge forward more. That is why we assess the area before treating.

How long does jelly roll Botox last?

Usually around three to four months. It starts working in 3 to 7 days and peaks at about two weeks.

Does Botox help dark circles or hollows under the eyes?

No. Hollows and dark circles are treated with under-eye filler or skin treatments, not Botox.

What is aegyo sal?

A Korean term for the small roll under the lower lashes when smiling, often seen as youthful and attractive. Some people keep or even enhance it; others prefer to soften it.
