If you only read one thing

Under-eye filler places a small amount of soft hyaluronic acid gel in the tear trough to fill a hollow that casts a shadow. Restylane Eyelight and Juvéderm Volbella XC are FDA-approved for under-eye hollows, and results last up to about a year. It suits people with a clear hollow, good skin and no puffy bags; it does not fix brown pigment or bags. Swelling settles within 2 weeks, and the filler can be dissolved if needed. At Glow Theory under-eye filler is $685.

  • Under-eye filler fills the tear trough hollow to remove the shadow it casts.
  • Restylane Eyelight (2023) and Juvéderm Volbella XC (2022) are FDA-approved for under-eye hollows.
  • Results last up to about a year, often longer.
  • Best for a clear hollow with good skin; not for puffy bags, fluid swelling or brown pigment.
  • Swelling and bruising settle within about 2 weeks; filler can be dissolved.
  • Rare vision changes or blanched skin need emergency care.

Under-eye filler (also called tear trough filler) places a small amount of soft hyaluronic acid gel in the hollow between the lower eyelid and the cheek. Filling that groove removes the shadow it casts, so you look less tired. It can look excellent in the right person, but the under-eye area is delicate and not everyone is a good candidate. Choosing the right patient, product and injector matters more here than almost anywhere else on the face.

This guide covers who under-eye filler suits, which fillers are FDA-approved for it, what treatment and recovery are like, how long it lasts, the risks and the cost at Glow Theory.

Who is a good candidate?

Good candidate

Filler tends to work well

  • The problemA visible hollow or groove that casts a shadow
  • SkinReasonably thick, with good elasticity
  • PuffinessLittle or no swelling or bags
  • TestDarkness improves when you look up into a light

Probably not

Other treatments fit better

  • Puffy bagsFat pushing forward; filler can make it look worse
  • Morning swellingFluid tends to collect, and filler holds water
  • Loose, crepey skinNeeds tightening rather than volume
  • Brown pigmentFiller fills shadows; it does not lighten color

If your dark circles are mostly pigment or thin skin, see our guide to dark circles under the eyes. Sometimes adding volume to the cheek is the better first step, because it lifts the lower lid area without putting filler under the eye at all.

Which fillers are used under the eyes?

FillerFDA approvalHow long it lasts
Restylane EyelightUnder-eye hollows, adults 21+ (2023)Up to about 12 months
Juvéderm Volbella XCUnder-eye hollows, adults over 21 (2022)Studied to 1 year

Both are soft, smooth hyaluronic acid gels designed for thin skin. Firmer fillers made for the cheeks are not used directly under the eye. Because they are hyaluronic acid, both can be dissolved with an enzyme if needed. Fillers that cannot be dissolved, such as Sculptra or Radiesse, are not used in the under-eye area.

What treatment is like

  1. Consultation: your injector looks at your under-eye area in different lighting, checks for bags, swelling and skin quality, and tells you honestly whether filler will help.
  2. Numbing: numbing cream is applied, and the filler contains lidocaine.
  3. Injection: many injectors use a blunt-tipped cannula through one small entry point, which lowers the risk of bruising and of entering a blood vessel; a fine needle is also used.
  4. A small amount: usually well under one syringe per side. Less is more under the eyes.
  5. Review: a follow-up visit, often at two weeks, lets your injector check the result and add a tiny amount if needed.

The whole appointment usually takes 30 to 45 minutes, and most people go back to normal activities the same day.

Recovery and results

  • Swelling: mild puffiness for a few days is common; under-eye swelling can take up to 2 weeks to settle fully.
  • Bruising: possible, especially with a needle; concealer covers it after a day.
  • Final result: visible at about 2 weeks, once swelling is gone.
  • How long it lasts: up to about a year, and often longer under the eyes, where there is little movement.

Risks and side effects

  • Common: swelling, bruising, tenderness and small lumps that settle within 2 weeks.
  • Bluish tint (Tyndall effect): filler placed too close to the surface can show through thin skin; it is dissolved.
  • Persistent puffiness: filler attracts water, and in some people the area looks puffy for months; dissolving fixes it.
  • Lumps or migration: uncommon with careful technique; dissolvable.
  • Blocked blood vessel: rare, but the area around the eye is where this matters most, since it can, very rarely, affect vision.

Seek help immediately if you notice any change in vision, eye pain, severe pain or skin turning white, gray, blue or mottled after under-eye filler. Call us straight away, and if we are closed, go to an emergency room and say you have had filler.

Under-eye filler is not given during pregnancy or breastfeeding, or to people with an active infection, recent eye surgery or certain eye conditions; tell us about any eye problems and medicines.
