Dark marks after acne are post-inflammatory hyperpigmentation (PIH): flat brown, gray or black marks caused by extra pigment made during inflammation. They are not scars, which are sunken or raised. Most fade in 6 to 12 months once breakouts stop, but deeper pigment and darker skin tones can take longer. Daily tinted SPF 30+ sunscreen with iron oxide, azelaic acid, retinoids, vitamin C and light peels speed fading, and treating acne prevents new marks.
- PIH is flat discoloration, not a scar.
- Red marks (PIE) come from blood vessels; brown marks (PIH) from pigment.
- Most marks fade in 6 to 12 months; deep pigment can take years.
- PIH is more common and longer-lasting in darker skin.
- Tinted sunscreen with iron oxide is the most important step.
- Azelaic acid, retinoids, vitamin C and peels speed fading.
- Avoid picking, harsh scrubs, lemon juice and imported whitening creams.

The flat brown, gray or dark marks left after a pimple heals are called post-inflammatory hyperpigmentation (PIH). They are not scars: the skin is smooth, and the color comes from extra pigment made during inflammation. PIH usually fades on its own in 6 to 12 months once breakouts stop, but it can last much longer in deeper skin tones or if acne keeps coming back. Sunscreen, brightening ingredients such as azelaic acid and retinoids, and gentle peels speed fading, and controlling the acne prevents new marks.
This guide explains how to tell dark marks from red marks and true scars, how long they last, what fades them, and what to avoid.
Dark marks, red marks or scars?
| Type | What it looks like | What helps |
|---|---|---|
| Dark marks (PIH) | Flat brown, gray or black marks; most common in medium to deep skin | Sunscreen, azelaic acid, retinoids, peels; time |
| Red or pink marks (PIE) | Flat red or purple marks from widened blood vessels; most visible on lighter skin | Time, sunscreen, BBL or vascular laser |
| Acne scars | Sunken pits or raised bumps; the texture is changed | Subcision, RF microneedling, lasers; see acne scar treatment |
A simple test: run a finger over the spot. If the skin is smooth, it is a mark that will fade; if it dips or bumps, it is a scar.
Why acne leaves dark marks
Inflammation from a pimple signals pigment cells to make extra melanin. When it settles in the top layer of skin, the mark is brown and fades in months. When it drops into the deeper layer, the mark looks gray-blue and can take years. Darker skin makes more pigment in response to inflammation, so PIH is more common, darker and longer-lasting in brown and Black skin. Picking, squeezing and sun exposure all make marks darker and slower to fade.
How to fade dark marks
- Daily sunscreenThe most important step. Use a broad-spectrum SPF 30+ tinted sunscreen with iron oxide, reapplied every two hours outdoors. Without it, other treatments struggle.
- Azelaic acidFades pigment and treats acne at the same time; gentle and suitable for most skin, including in pregnancy with your doctor’s approval.
- RetinoidsAdapalene or tretinoin speed skin turnover and prevent new breakouts.
- Vitamin C, niacinamide and kojic acidBrighten and even tone; see vitamin C for skin.
- HydroquinonePrescription strength, used for limited periods under a dermatologist’s guidance; the FDA advises against most over-the-counter hydroquinone.
- Chemical peelsA series of light salicylic or glycolic peels speeds fading; see peels for dark spots.

What to avoid
- Picking and squeezing pimples, which deepens and lengthens marks.
- Harsh scrubs and strong, stinging products, which inflame skin and can make marks darker.
- Lemon juice, bleach and imported “whitening” creams, which can burn skin or contain mercury or hidden steroids.
- Aggressive lasers and strong peels on darker skin without an experienced provider, as they can cause new dark or light patches.

Frequently asked questions
Are dark marks from acne permanent?
Usually not. Most fade in 6 to 12 months once breakouts stop, though deep marks can take longer.
What is the difference between PIH and acne scars?
PIH is flat discoloration; acne scars change the skin's texture, leaving pits or raised bumps.
What fades acne dark spots fastest?
Daily tinted sunscreen plus azelaic acid or a retinoid, with light peels for stubborn marks. Prescription hydroquinone may be used short term.
What are red marks after acne?
Post-inflammatory erythema (PIE), caused by widened blood vessels. They fade with time and can be treated with BBL or vascular lasers.
Why do my dark spots get darker in summer?
Sun and visible light stimulate pigment. Tinted sunscreen with iron oxide helps block both.
Can lasers treat PIH on dark skin?
Gentle options such as MOXI can, with careful settings. Aggressive lasers can make pigment worse on darker skin.
