Why the face loses volume with age, menopause and weight loss, where it shows first, and how hyaluronic acid fillers, Sculptra and Radiesse restore it, plus what face yoga can do and costs in Carlsbad.
How we write and check our articles

If you only read one thing
Facial volume loss happens as fat pads shrink and slide down, facial bones lose height and collagen thins, speeded up by menopause, sun, smoking and weight loss. It shows as hollow temples, flat cheeks, under-eye hollows, deeper smile lines and jowls. Hyaluronic acid fillers restore volume immediately for 6 to 24 months, while Sculptra rebuilds collagen gradually over 2 to 3 sessions for results lasting up to 2 years.

- Volume loss shows in the temples, under-eyes, cheeks, smile lines and jaw.
- Hyaluronic acid fillers give immediate, reversible results lasting about 6 to 24 months.

- Microneedling and lasers improve skin quality but do not replace lost volume.

A face that looks tired, sad or older than you feel is often not about wrinkles at all. It is about volume. From our thirties onward, the fat pads that keep cheeks full shrink and slip downward, the bones of the face slowly lose height, and collagen thins. The result is hollow temples, flatter cheeks, under-eye hollows, deeper folds around the mouth and a softer jawline.
This guide explains why facial volume loss happens, where it shows first, and which treatments actually restore volume, from hyaluronic acid fillers to collagen stimulators such as Sculptra, plus what facial exercises can and cannot do.
Why the face loses volume
- Fat pads shrink and shift: the face has separate fat compartments. With age, some shrink (around the temples, eyes and upper cheeks) and others descend, which deepens the folds from nose to mouth and creates jowls.
- Bone changes: the eye sockets widen and the cheekbones and jaw lose height and projection over decades, so there is less support for the soft tissue on top.
- Collagen and elastin loss: skin becomes thinner and less springy. The American Academy of Dermatology notes that women lose about 30% of skin collagen in the first five years of menopause, then about 2% a year.
- Sun and smoking: UV light and smoking break down collagen faster.
- Weight loss: losing a lot of weight, especially quickly, as can happen with GLP-1 weight loss medicines, often takes fat from the face too.
- Genetics and lifestyle: bone structure, very low body fat from endurance training and illness all play a part.
Where volume loss shows
| Area | What you notice | Common treatment |
|---|---|---|
| Temples | Hollowing at the sides of the forehead | Filler or Sculptra |
| Under the eyes | Hollows and shadows that make you look tired | Soft under-eye filler, if the skin is suitable |
| Cheeks | Flatter midface, less lift | Cheek filler (Voluma, Lyft) or Sculptra |
| Nose to mouth | Deeper smile lines | Cheek support first, then line filler |
| Mouth and jaw | Marionette lines, jowls, a less defined jaw | Chin and jawline filler, Sculptra |
A good injector treats the cause rather than each line. Restoring cheek support, for example, often softens smile lines and under-eye shadows at the same time.

Hyaluronic acid fillers
Hyaluronic acid is a sugar that occurs naturally in skin. Fillers made from it come in different firmness levels: firmer gels such as Juvéderm Voluma and Restylane Lyft lift the cheeks and midface, while softer gels such as Restylane Eyelight suit the under-eye area. Results are visible straight away, with some swelling for a few days. Because hyaluronic acid filler can be dissolved, it is the most adjustable option. See dermal fillers in Carlsbad.
Sculptra
Sculptra is a poly-L-lactic acid injectable that stimulates your own collagen over the following weeks. It was first FDA-approved in 2004 and, in 2023, was approved for cheek wrinkles, with 94% of patients still improved at two years in the study behind that approval. Most people need 2 to 3 sessions about a month apart, and results build gradually, so it suits people who want a subtle, natural change. See Sculptra in Carlsbad.
Radiesse
Radiesse (calcium hydroxylapatite) gives immediate lift and also stimulates collagen. It is often used along the jawline, chin and cheeks, and for the hands. It cannot be dissolved like hyaluronic acid filler.
Treatments that improve skin, not volume
Microneedling, Morpheus8 and laser resurfacing improve skin texture, firmness and fine lines, and pair well with fillers, but they do not replace lost volume. If hollows are the main concern, volume comes first.
Do facial exercises or face yoga work?
There is a little evidence. In a small 2018 Northwestern University study, 16 women aged 40 to 65 who did 30 minutes of facial exercises daily for 8 weeks, then every other day for 12 weeks, had fuller upper and lower cheeks, and their estimated age dropped by almost three years. The study was small and needed a lot of commitment, and exercises cannot rebuild lost fat or bone. Facial exercises are safe to try, but they will not match the effect of filler or Sculptra.
What about hormone therapy?
Estrogen helps maintain collagen, and skin often changes quickly around menopause. Menopausal hormone therapy is a medical decision made with your doctor for symptoms such as hot flashes, weighing its benefits and risks; it is not prescribed to treat facial volume loss on its own. Daily sunscreen, a retinoid and in-office collagen treatments help whatever you decide.

Safety: choose your injector carefully
Fillers are FDA-regulated medical devices. Rare but serious complications, such as filler blocking a blood vessel, need immediate treatment, so choose a medical provider who uses FDA-approved products from licensed suppliers and has filler dissolver on hand. The FDA warns never to buy fillers online or have them injected by unlicensed people. Bruising and swelling for a few days are common; tell us about blood thinners, cold sores and any past filler before treatment.

Frequently asked questions
What causes facial volume loss?
Shrinking and shifting fat pads, loss of facial bone height, and thinning collagen and elastin, made faster by menopause, sun, smoking and weight loss.
What is the best treatment for facial volume loss?
Hyaluronic acid fillers for immediate, targeted volume, or Sculptra for gradual, natural-looking volume across the face. Many people use both.
How long do fillers for volume loss last?
Hyaluronic acid fillers last about 6 to 24 months depending on the product and area; Sculptra results can last up to 2 years.
Can facial exercises restore volume?
A small 2018 study found daily facial exercises made cheeks look slightly fuller over 20 weeks, but exercises cannot replace lost fat or bone.
Does weight loss cause facial volume loss?
Yes. Losing a lot of weight, especially quickly, often reduces facial fat and can make the face look older or hollow.
Is Sculptra or filler better?
Filler gives instant, adjustable lift in specific areas; Sculptra builds collagen gradually for overall volume. Your injector may recommend one or a combination.
