Depressed Acne Scars: Ice Pick, Boxcar and Rolling Scars, and What Treats Each
How to tell ice pick, boxcar and rolling acne scars apart, which treatments work for each (subcision, Morpheus8, lasers, TCA CROSS, fillers), realistic results and costs in Carlsbad.

If you only read one thing
Depressed (atrophic) acne scars are dents left when deep acne destroys collagen. There are three types: narrow, deep ice pick scars; wider, sharp-edged boxcar scars; and broad, wavy rolling scars. Ice pick scars respond to TCA CROSS or punch excision, boxcar scars to laser resurfacing, RF microneedling or filler, and rolling scars to subcision. Most people need a combination of treatments over several months, after their acne is controlled.
- 80 to 90% of acne scars are depressed (atrophic); most people have a mix of types.
- Ice pick scars are narrow and deep; they need TCA CROSS or punch techniques, not lasers alone.
- Boxcar scars are wider with sharp edges; lasers, RF microneedling, TCA CROSS or filler help.
- Rolling scars are tethered down by fibrous bands; subcision releases them.
- Control active acne before treating scars.
- Improvement builds over 3 to 6 months; complete clearing is rare, but scars can become much less noticeable.

Depressed acne scars, also called atrophic scars, are the dents and pits left behind when deep breakouts destroy collagen faster than the skin can rebuild it. They make up the large majority of acne scars, and they come in three main shapes: ice pick, boxcar and rolling. The shape matters, because each type responds to different treatments, and most people have a mix of all three.
Depressed scars do not disappear with creams alone, but the right combination of treatments can make them much less noticeable. This guide explains how to tell the types apart, which treatments work for each, what results are realistic and what treatment costs at Glow Theory.
Why depressed scars form
A deep, inflamed breakout damages the skin around the pore. As it heals, the body does not replace all the lost collagen, so the surface sinks. In some scars, bands of fibrous tissue form underneath and tether the skin down, which creates a rolling, wavy look. The more inflamed the acne, and the longer it lasts, the higher the risk. Picking and squeezing spots adds to the damage.
Depressed scars are different from the flat red or brown marks many people call “scars.” Those marks (post-inflammatory redness and hyperpigmentation) are not indented and usually fade over months with sun protection and skincare.
The three types of depressed acne scars
| Type | What it looks like | Treatments that work best |
|---|---|---|
| Ice pick | Narrow (under 2 mm), deep pits, like a large open pore | TCA CROSS (a strong acid placed in the scar), punch excision; lasers alone do little |
| Boxcar | Wider, round or oval dents with sharp edges, like a chickenpox scar | Shallow: laser resurfacing, RF microneedling, peels. Deep: punch elevation, TCA CROSS, filler |
| Rolling | Broad, shallow dips with soft edges that make skin look wavy, worse in side lighting | Subcision to release the tethering bands, then RF microneedling, lasers or filler |
A simple test: stretch the skin gently. Rolling and shallow boxcar scars soften when stretched; ice pick and deep boxcar scars stay visible.
Treatment options for depressed acne scars
Subcision
A fine needle is passed under the scar to cut the fibrous bands pulling it down. The skin lifts, and the controlled injury prompts new collagen. It is the treatment of choice for rolling scars and helps many boxcar scars, but not ice pick scars. It is done under local numbing; bruising for a week or so is common. Most people need 1 to 2 sessions. See acne subcision in Carlsbad.
RF microneedling (Morpheus8)
Fine needles deliver radiofrequency heat below the surface to remodel collagen. It improves rolling and shallow boxcar scars and overall texture, and the American Academy of Dermatology describes RF microneedling as safe for all skin tones. Plan on 3 to 4 sessions about 4 to 6 weeks apart, with results building over 3 months after the last one. Read what Morpheus8 is.
Laser resurfacing
Fractional lasers remove or heat tiny columns of skin to smooth the edges of boxcar scars and improve texture. HALO is a gentler hybrid laser with 1 to 2 days of downtime, best for mild scarring and texture on lighter to medium skin. Erbium laser resurfacing goes deeper for more significant scarring, with about 10 to 15 days of downtime. Ablative lasers carry a higher risk of dark or light patches on darker skin, so your skin type guides the choice; see our Fitzpatrick skin type guide.
TCA CROSS and punch techniques
For ice pick and deep, narrow boxcar scars, a high-strength trichloroacetic acid (TCA) is applied precisely into each scar to rebuild collagen from the base, usually over several sessions. Punch excision removes a deep scar and closes it with a stitch; punch elevation lifts a boxcar scar’s floor to the surface.
Fillers and collagen stimulators
Filler can lift soft rolling and boxcar scars right away. A PMMA filler (Bellafill) is FDA-approved for moderate to severe acne scars on the cheeks; hyaluronic acid fillers and Sculptra are also used and gradually wear off. Fillers work best after subcision has released the scar.
Chemical peels
Medium-depth chemical peels improve mild scarring, texture and the dark marks that often sit alongside scars. They are a supporting treatment rather than a fix for deep scars.
A typical treatment plan
- Control active acne with skincare or prescription treatment, and keep it controlled.
- Map your scars at a consultation that includes a VISIA skin analysis, so each scar type gets the right tool.
- Release and lift rolling scars with subcision, and treat ice pick scars with targeted techniques.
- Resurface and remodel with a series of Morpheus8 sessions or laser resurfacing.
- Fill what remains with filler if needed, and keep up sunscreen and a retinoid at home.
Collagen remodeling is slow: most improvement shows 3 to 6 months after a series, and it can keep improving for up to a year. Honest expectations matter. Experts note that complete clearing of acne scars is the exception rather than the rule; the goal is scars that are clearly softer and less noticeable, especially in everyday lighting.

Can you prevent depressed acne scars?
- Treat acne early, especially deep, painful or cystic breakouts, which are the ones that scar.
- Do not pick or squeeze spots.
- Wear sunscreen daily so marks fade faster and scars do not darken.
- Use a retinoid to keep pores clear and support collagen; see our retinol guide.

Frequently asked questions
What are depressed acne scars?
Indented (atrophic) scars left when deep, inflamed acne destroys collagen. They include ice pick, boxcar and rolling scars.
What is the best treatment for depressed acne scars?
It depends on the type: subcision for rolling scars, TCA CROSS or punch excision for ice pick scars, and laser resurfacing, RF microneedling or filler for boxcar scars. Most people do best with a combination.
Can depressed acne scars go away on their own?
No. Flat red or brown marks fade over time, but indented scars need treatment to improve.
How many sessions do acne scars need?
Subcision often takes 1 to 2 sessions, RF microneedling 3 to 4, and TCA CROSS several. Your plan depends on your scar types.
Does Morpheus8 work on acne scars?
Yes, especially for rolling and shallow boxcar scars and overall texture, usually over 3 to 4 sessions.
Is acne scar treatment safe for darker skin?
Subcision, RF microneedling and microneedling are considered safe for all skin tones. Ablative lasers carry a higher risk of pigment changes, so they are chosen carefully.
